Asthma – Case Management Assistant/ Medical Assistant

Here is a great Monday-Friday, 8:30 a.m. - 5 p.m. position in the healthcare field. The Case Management Assistant/ Medical Assistant works with primary care providers, clinic staff and other collaborative teams to assist in the identification basic needs, organize activities, and collaborate with other health care team members to coordinate and implement plans of care for patients. Case management activities will include services for women that are pregnant and/or have chronic conditions including their families in the effort of linking them with health and community services/partners that will promote self-management, improve health outcomes and reduce disparity.

This is a full-time position, at a growing and expanding clinic. This is a very long-term contract position with benefits like medical insurance, dental insurance, 401K, PTO and paid sick time and training.

• Outreach to Pediatric patients with asthma
• Outreach to Adult patients with asthma
• Refer patients determined by Case Management Assistant/ Medical Assistant outreach and discussion with patient/caregiver who needs an updated or new asthma action plan, who have unmanaged asthma, or who need asthma education to their PCP or specialist for additional asthma management as a result of the outreach.
• Refer patients who meet county-specific eligibility requirements and have asthma education and trigger mitigation needs to Asthma Home Based Services.
• Identify and mitigate cultural barriers for patients identified as having cultural barriers to asthma management, cultural-based gaps in asthma knowledge, or culturally based misconceptions around asthma management, etc.
• Assesses, plans, and coordinates patient care according to established clinic policies and procedures, objectives, performance improvement activities and safety, as well as environment and infection control standards.
• Assists patients and their families with the identification, selection, monitoring, and documentation of self-management goals. Follows up with the patient as determined by the care coordination plan.
• Promotes autonomy and healthy behaviors in identified patients by building on the strengths of the patient and support system.
• Plans, coordinates, and provides patient-specific health education as determined from the assessed care needs.
• Work with the patient, family members, support system, providers, care manager and health care team members to develop and implement an individualized clinical plan of care for the patient.

• Establishes program goals and objectives for asthma. Modifies the plan of care as needed.
• Identifies resources and provides assistance to help clinics implement asthma management.

Education and Experience:

Current Minnesota Community Health Worker certification OR

Completion of Medical Assistant Program

Licenses and Certifications:

Certification in CPR desired

Pay: $22.00 - $26.00 per hour
