The Population Health Manager plays a pivotal role in supporting the CDM Nursing staff, PHMP Coach staff, and AHDI Leadership. This position involves overseeing day-to-day care management activities, providing functional guidance to the nurses and coaches, and ensuring optimal outcomes. The successful candidate will maintain necessary controls to identify performance deficiencies, drive PMO projects, and drive new product and service initiatives. Additionally, they will uphold the quality of the company's offerings, consistently meeting or exceeding customer expectations. The Population Health Manager collaborates directly with AHDI Leadership to manage the overall flow of activity within AHDI.
