We are currently looking for a Home Health Case Manager RN that is interested in joining our team!

Job Requirements and Responsibilities:

• Restores and promotes patient's health by completing the nursing process.
• Assures quality of care by measuring health outcomes against patient care goals
• Maintains safe and clean working environment by complying with procedures, rules, and regulations.
• Completes initial assessment of patients to determine home care needs as well as regular recertifications.
• Initiates the plan of care and makes necessary revisions.
• Responsible for the completion of all home health documentation, including admissions, recerts, discharges, and all OASIS documentation.
• Conducts supervisory visits when necessary to monitor home health aide performance.
• Serve as case manager for Medicaid patients.

Qualifications/Skills:

• Registered Nurse (RN), required
• Clinical skills
• Case manager experience, recommended

Job Type: Full-time

Pay: From $40.00 per hour
