Day shift hours with no weekend requirement

We are seeking a Home Health Admissions RN/Care Coordinator for Lawrence County and the surrounding areas. The purpose of the care coordinator is to help maintain stabilization, delay/prevent deterioration of health status, manage chronic conditions, and/or improve health status for patients receiving this service.

Job Requirements and Responsibilities:

• Performs medical coordination to manage the healthcare of the individuals receiving this service including physician consults, medication and supply ordering, and helping patients understand new or existing diagnoses.
• Development and oversight of a healthcare support plan.
• Required to coordinate and collaborate with other care managers and healthcare professionals to ensure quality care management is being delivered.
• Deliver care coordination through weekly consultations or reviews.
• Face-to-Face visits with the patient at a minimum of one visit per month
• Completes an initial OASIS assessment of patient and family to determine home care needs.
• Works with physician and/or referral source to obtain all pertinent clinical information for optimal assessment generation
• Initiates the plan of care which establishes patient-centered goals as approved by physicians.

Qualifications/Skills:

• Registered Nurse (RN), required
• 1 year experience in a healthcare setting
• Medicaid Home Health experience, recommended
• Proficient knowledge of Microsoft Office and EMR systems
• Valid Driver's License
• Must pass a background check

Job Type: Full-time

Pay: From $37.00 per hour
