Keetoowah Cherokee Treatment Services is Hiring a Case Manager Near Tahlequah, OK

Salary: $18- $20 per hour
Job summary:
We are looking for an experienced Case manager to join our team. Case managers assess the needs of the individual and/or family and coordinate care services. This can include nutrition, rehabilitation, transportation, and/or educational services. Case managers then monitor the continuity of care and serve as an advocate for the individual. Case managers coordinate services and connect clients with resources that can help them with their challenges, whether they be psychiatric illness, substance abuse or other personal problems that impact their ability to function normally in everyday life. Responsibilities of a behavioral health case manager in our grants department typically include:
Responsibilities
? Cultivating relationships built on trust, respect, empathy, mutual experience, and
choice.
? Offering support through validation, encouragement, empowerment, and
highlighting strengths.
? Skills in interpersonal and human relations, including teaming and collaboration
abilities.
? Recognizing a person's personal beliefs, cultural values, previous experiences,
and individual needs.
? Providing education about recovery, advocating for oneself, and how to navigate
treatment or the healthcare system.
? Assisting during crises by discussing warning signs, healthy coping, and sharing
resources.
? Communicate patient needs to a variety of care team members and follow up
accordingly.
? Work collaboratively with patients, families, therapists, and psychiatrists to
ensure high quality care.
? Act as the patient's advocate as it relates to insurance coverage and financial
assistance.
? Maintain the patient's comprehensive clinical record through detailed
documentation.
? Help clients achieve wellness and autonomy.
? Help clients with community resources like a food pantry, job assistance center,
social services, transportation, navigating online bill payment, or health clinic.
? Ability to work with a team to achieve goals and objectives. Keep detailed
progress notes.
? Complete all paperwork for each patient and have it ready for the billing
department daily.
? Represents the Echota Behavioral Health in the fostering of referral
development.
? Ability to utilize a computer, MS Windows/Office, exercise sound independent
judgement in establishing work priorities.
? Ability to meet assignment deadlines.
? Ability to maintain a confident and professional demeanor.
? Ability to maintain patient and employee confidentiality via federal HIPAA
regulations.
? Attends administrative and clinical staffing when scheduled.
Requirements
? Bachelor's degree
? Certified Case Manager I or II
? 1-3 years of clinical experience, preferably in case management, or related
experience
? Ability to walk and stand 50% of the time.
? Proficient computer skills including experience with Microsoft Excel
? Strong written and verbal communication skills
? Excellent clinical assessment and analytical skills
? Follow all guidelines, regulations, standards, and Federal state and tribal
policies.
? Must possess a valid Oklahoma Driver?s license and successfully complete all
pre-hire requirements.
UKB and Tribal preference applies.
