Overview:
We are seeking a professionally driven, highly organized and meticulous individual to join our team as a Care Coordinator. As a Care Coordinator, you will play a crucial role in providing administrative support and coordinating care for our clients. The Care Coordinator works in collaboration and continuous partnership with clients some who are chronically ill. Clients reside in private home settings. The position will require frequent visits and an average of twenty or more on- site interactions per month. This is an excellent opportunity for someone with experience in medical administrative support and a passion for helping others.
Must travel up to a 50- mile radius of the home office utilizing both private and company owned vehicles. Must hold a valid driver's license and dedicated safe vehicle. Travel is required in all seasonal weather conditions. Accompanying and transporting clients to appointments is also required.
Summary of Duties:
- Coordinate client care by scheduling appointments, arranging referrals, and ensuring timely follow-up.
-Create and adhere to the care plan, developed in the coordination process.
- Maintain accurate and up-to-date client records and documentation
- Assist with long term healthcare insurance claims processing
- Provide exceptional customer service to clients, addressing inquiries and concerns
- Collaborate with healthcare providers to ensure seamless communication and coordination of care
-Maintain continuity of care by managing relationships with professional peers, and referral sources.
- Manage office tasks such as answering phone calls, scheduling appointments
-Adhere to company policies, support the company culture by positive teamwork.
Experience:
- 2 plus years of previous experience in medical administrative support or related field.
-CPR certification
-Experience with the frail elderly population
-Familiarity with medical terminology, medications, and health procedures
-Basic nutritional needs
-Basic oral health needs
- Proficient in office software applications (e.g., Microsoft Office Suite)
- Knowledge of medical office procedures are a plus
-HIPAA training
