JOB
Position SummaryPrimary initial responsibilities for this position include coordination of agency Emergency Preparedness activities, including writing and maintaining agency plans, following federal and state grant guidelines. Additional responsibilities include working Women, Infants and Children (WIC) clinic and participating in Disease Prevention and Control (DPC) activities. Candidate must be detail oriented, flexible, and have strong collaboration skills. Must also be passionate about writing, implementing, and teaching regarding emergency response plans. Responsibilities may require flexible work hours.This position provides services necessary to support, maintain, and implement public health programs within assigned areas. Nurse duties fall within the core functions of public health assessment, policy development, and assurance through work in homes, schools, the broader community, and target populations. Nurse provides leadership in the implementation and provision of public health programs in the county.
EXAMPLE OF DUTIES
Essential Duties and Responsibilities: Receives referrals and assesses clients and families for needed services. Assesses home environment for safety. Develops and implements a plan of care with client/family. Monitors progress of client and adjusts plan of care as needed. Prepares and completes clinical records and maintains accurate client records and dailies. Nurse may make home visits according to the nurse’s health assessment and plan of care. Provides client/family health education and case management as needed.Orients and instructs other staff members and provides a framework and guidance for contracted services.Supervises paraprofessional staff and delegated nursing functions.Refers clients and family members to appropriate agency and community resources. Administers and documents immunizations to children and adults per agency protocols. Provides vaccine related education to promote completion of age-appropriate immunizations. Administers TB screenings to high-risk families, groups, contracted services, county employees, and students. Conducts follow up education and monitoring for those with positive results or high risk contacts. Provides case management and directly observed therapy for TB referrals.Assists the Minnesota Department of Health with communicable disease outbreaks and investigations within the county. Responds to public health emergencies and assists with public health preparedness planning. Provides contracted school health services, including vision, hearing, growth and development, and early childhood screening.Provides health and nutrition education services in WIC.Coordinates agency programs established to meet an identified need in the community.Plans and coordinates health education and presentations.Provides input to Agency leadership in planning, coordinating, and participating in the development of health care services for county residents with other agencies/entities.Serves on various health committees to coordinate services, provide information, or share professional knowledge with respect to specific programs/services. Participates as requested in grant writing and grant program coordination.Performs other related work duties as apparent or assigned.Additional Essential Duties and Responsibilities for Program Coordinators: Develop, implement, and provide on-going evaluation for the program. Responsible for staff scheduling (if applicable) and making staffing recommendations to the Administrative Team. Problem solves staffing issues or conflicts. Utilize administration as appropriate.Responsible for program quality assurance measures, stay current on best practices in the program area, plan for and implement program changes based on program and best practice study. Consult with and receive support from Agency Administration when making/recommending programmatic changes. Responsible for coordinating with and interacting with other agency programs and community agencies/programs to provide coordinated efforts to improve the public’s health. Coordinate with agency activities to meet an identified need in the community.Responsible for community assessment, problem identification and planning surrounding their focus area of public health practice. Responsible for collecting and maintaining all data required for reporting and internal documentation. Responsible to file budget and programmatic reports in a timely and accurate manner as required of the program.Conducts program services to meet all rules and regulations (local, state, and federal) to remain in compliance with program standards and best practices. Meets regularly with Supervisor to discuss and report on goals, challenges, successes, and recommendations for programmatic changes needing approval. Responsible for setting a budget for the program, tracking expenses and financial reporting in conjunction with their supervisor and, if required, the Director. Represent the Agency and program on various committees and in the community to enhance the public’s health. Represent the Agency and program in professional and community activities.Perform other related work duties as apparent or assigned.
SUPPLEMENTAL INFORMATION
Knowledge: Knowledge of nursing skills: Nursing process, technical nursing procedures, good communication skills (written and oral), staff supervision, knowledge of community resources, networking with other human services personnel, ability to work independently, adjust to changes, and ability to delegate. Knowledge of principles of Public Health. Knowledge of function of other community health resources and of social service agencies. Knowledge of time management and scheduling day independently. Skills and Abilities: Ability to make independent decisions.Ability to utilize resources for information to meet client needs. Flexibility, willingness to seek help or accept help. Proficient in basic computer skills and willing to learn new computer skills as the technology changes.Additional Knowledges Abilities, and Skills for Program Coordinators: Strong leadership skills. Ability to establish and maintain effective working relationships with all segments of the staff, community, and professional field. Coalition building skills if applicable. Excellent communication skills (written and oral), staff oversight and coordination, knowledge of community resources, network with other personnel, ability to work independently, adjust to changes, and ability to delegate. Extensive knowledge of principles of Public Health. Knowledge of other community health resources and of social service agencies.
