Essential Duties and Responsibilities:

• The Admissions Coordinator/Charge Nurse oversees patient admissions and works with the nursing team in providing care.
• Provides clinical liaison services between short-term sub-acute inpatient facility and referring hospitals. Works closely with hospital case managers/discharge planners to identify appropriate patients for the sub-acute facility.
• Makes in-person visits to hospitals to meet with case managers and potential patients.
• Provide direct patient care in accordance with established nursing care plans.
• Assist in training new staff.
• Ensures compliance with health and safety regulations
• Administer medications and treatments as prescribed by the physicians.
• Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
• Maintain accurate documentation.

Minimum Qualifications:

• Must hold and maintain a current license to practice as a Licensed Practical Nurse or Registered Nurse in the State of Hawaii.
• Recent clinical experience, education, or specialty skills specific to geriatrics.
• Demonstrate understanding that state and Federal rules and regulations govern the practices within the facility.
• Clinical assessment and chart review experience is required.
• Recent clinical experience, education, or specialty skills specific to geriatrics.
• Must be self-motivated.
• Strong communication and interpersonal skills to effectively interact with patients, families, teammates, and healthcare professionals.
• Ability to prioritize tasks and manage time effectively.
• Basic computer knowledge and ability with an aptitude to learn company software.
• Must be capable of maintaining regular attendance.
• Must meet all local health regulations and pass post-employment physical exam, if required. This requirement also includes drug screening, criminal background investigation and reference inquiry.

Job Type: Full-time

Pay: $75,000.00 - $90,000.00 per year
