- Practice Setting: SNF (Skilled Nursing Facility)
- On-call: No
- Conduct comprehensive physical therapy assessments and develop individualized treatment plans for patients.
- Implement evidence-based therapeutic interventions to enhance patients' functional abilities.
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
- Maintain accurate and up-to-date patient records and documentation.
- Monitor and report patient progress to healthcare professionals and family members.
- Valid PT license in the state of Texas
- Strong clinical and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Availability to start ASAP
