JSAS HealthCare, Inc is a non-profit outpatient Opioid Treatment Program (OTP) located in Neptune, NJ. JSAS HealthCare, Inc has been providing medication assisted treatment services to Monmouth and Ocean County residents since 1984.

Position Summary:

The Nurse Case Manger works as part of a multidisciplinary team to provide medical case management services.

This is a full-time position Monday – Friday

Responsibilities:

Complete health screening/nursing assessment upon admission and review on an ongoing basis to identify diagnoses or risk for Hepatitis C and other medical diagnoses.

Develop an individualized patient care plan based on screening and medical assessments.

Provide education to patients on how to follow their care plan.

Evaluate and determine needs of patients as needed.

Review results from medical tests with patients.

Educate patients on Hepatitis C and new medication available.

Assist patients in navigating the health care system. Coordinate Specialty care, follow-up on test results and other care coordination needs.

Requirements:

Licensed as an RN in New Jersey
