Acclaim PhysicianGroup, working in conjunction with Family Health Medical Home of Tarrant County, is currently lookingfor an Advanced Practice Provider to join our FHC Center. This is a full time position, providing primarycare services to patients experiencing homelessness or experiencinghigh-utilization.
Job Summary:
The Advanced Practice Provider practices under the directsupervision of the supervising physician and is responsible for assisting withall aspects of patient care, including diagnosis, treatments and consultationswithin the outpatient setting. Thisposition will also educate patients about preventative care and prescribedtreatments; may conduct physicals, order tests and serve as a patient'shealthcare provider.
Full spectrum (adult & children). Patients also have mental healthand substance abuse issues. We are looking for a compassionate provider who isnot afraid of a challenge and has experience working with a similar population.This is a great opportunity to work with complex medical, behavioral and socialissues, and to provide compassionate, quality care.
Schedule:3 x 12-hour shifts per week
Duties and EssentialJob Functions:
-Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.
-Reviews patient medical charts.
-Makes appropriate patient referrals as necessary.
-Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
-Provides care education to patients and families.
-Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.
-Maintains a collaborative environment with providers, clinical staff, and support staff.
-Supports academic, educational and research endeavors.
-Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.
-Understand and fully incorporate themselves into the broader homeless services community through their health care role.
-Performs other job related duties as assigned.
