Summary: To provide an individualized plan of care for each Member using effective and efficient case management techniques.
Reports To: Program Director, Senior Choice at Home
Titles Supervised: None
DUTIES
Essential Responsibilities:
1.Schedule and complete pre-admission assessments of prospective Members which may include a mini-mental exam, depression screening, nutritional and home assessments and level of care guidelines (ADL's/IADL's), etc.
2.Manage a caseload of Members, including regular communication with Members and their families, assessments of needs, coordinating and scheduling services.
3.Initiate the Member's Care Plan and maintain detailed records of all Member activity in the Member's file and the REPS system.
4.Activate care coordination for Members and supervise the provision of care.
5.Develop and update a community referral directory and resource files for Members.
6.Develop and implement wellness programs for Members.
7.Represent Senior Choice at Home at professional meetings, networking opportunities, community organization meetings, etc.
8.Complete billing records for Member caseload each month, and other tasks as assigned.
9.Other related duties as assigned.
QUALIFICATIONS
Minimum Qualifications:
RN/Social Worker or other relevant experience and education
Experience: Minimum 2-3 years' experience working with seniors in the healthcare environment, home care experience a plus
Licensure: Valid Connecticut Drivers License
Knowledge/Skills/Abilities:
Ability to handle multiple demands and deadlines; Ability to communicate effectively; Ability to utilize various software packages to carry out position responsibilities. Knowledge of community-based services for the elderly.
