Jewish Family & Children's Service is Hiring a Treatment Coordinator (Behavioral Health) Near Phoenix, AZ

Jewish Family and Children’s Service’s (JFCS) is among the most long-standing providers of behavioral health and integrated health services in Maricopa County. Treatment Coordinator: The Treatment Coordinator provides ongoing care-coordination services to assist in addressing the client and family’s ongoing service needs, including both primary care and behavioral health. This position assists clients and families in obtaining community support, identifying resources, and resolving barriers.

Duties include:

  • Performs ongoing coordination of services to children and adults in accordance with the treatment plan.
  • Responds to initial crisis calls and identifies needs & necessary resources accordingly.
  • Uses established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services while demonstrating a positive, strengths-based attitude of teamwork and shared leadership.
  • Provides coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.

Qualifications:

  • Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered
  • Relevant experience working in a behavioral-health setting
  • Level 1 Fingerprint Clearance Card
  • Verification of education, employment, references
  • MVR matching JFCS driving guidelines
  • AZ Driver's License and Valid Auto Insurance
