If so, we invite you to join our team as an AgeWell Responder and Outreach Specialist.
As an AgeWell Responder, you will play a vital role in responding to the AgeWell phone line, providing a full spectrum of services to older adults and their families. Your responsibilities will include offering information, referrals, and consultations, contributing to the development of Assessment and Care Plans, and actively participating in the ongoing care management for clients. Additionally, you will have the opportunity to engage with the community by conducting presentations, workshops, and psycho-educational groups targeting the older adult population and their caregivers.
Key Responsibilities:
Customer Service Excellence: Respond professionally and timely to external and internal requests for clinical services through various channels.
Provide Information and Referral (I&R) services to individuals not ready for a full consultation or those not eligible for specific services.
Respond appropriately to urgent situations, demonstrating care for both clients and non-clients.
Client Assessment and Consultation: Conduct consultations with older adults and their families to assess needs and family dynamics.
Enter clients into the Carelogic/ECR System, ensuring accuracy in information and documentation. Provide brief and full in-person consultations to address client and caregiver needs effectively.
Support and Resource Provision: Develop and maintain a collection of pertinent resource materials for distribution.
Demonstrate a working knowledge of agency and community services to meet the diverse needs of older adults and their caregivers.
Program Marketing and Outreach: Engage in community outreach activities, making presentations, and conducting workshops and psycho-educational groups.
Measure mission/client engagement levels through effective marketing strategies.
