We are currently looking for RN's for our FIVE STAR RATED skilled Home Health program serving Allegheny, Beaver, Butler, Westmoreland and Washington counties including areas such as Pittsburgh City / South Park / South Hills / Moon / Coraopolis / Beaver / Ambridge / Charleroi / Belle Vernon / Canonsburg.

Job Summary for Home Health RN Case Manager:

The Home Health Registered Nurse Case Manager is a responsible as a member of the interdisciplinary group who works under the general direction of the Medical Director and/or the attending physician in the provision of skilled nursing assessment, planning and coordination of services in order to maximize the comfort and health of patients and families.

Required Education, Training and Experience for Home Health RN Case Manager:

• Graduate of a school of nursing accredited by the National League of Nursing, Baccalaureate degree in nursing preferred.
• Registered nurse in the State of Pennsylvania.
• Pennsylvania driver's license, current auto insurance and access to a car during working hours.
• Home Health experience is preferred.
• RN Case Manager Home Health
• Required Skills and Abilities for Home Health RN Case Manager.
• Demonstrate flexibility, excellent assessment skills, ability to supervise others and the confidence to work independently.
• Communicate effectively within a team.
• Manage time efficiently.
• Show concern for the provision of quality services.
• Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles.
• Ability to do public speaking, educational programs and community outreach.
• Ability to maintain confidentiality of information
• Basic Computer Skills
• Ability to lift a minimum of 50 pounds.
• Ability to push/pull a minimum of 50 pounds.
• RN Case Manager Home Health
• RN Case Manager in Home Health, Palliative Care, or Long Term Care experience a plus.
