The Case Manager is typically an LVN with ensuring that patient care is coordinated and managed appropriately;
Case Managment is incharge of staffing and plotting visits for SN (skilled nurses), MSW, CHHA, call patients after SOC/RECERT/ROC, Assign Supervisory visits for LVN’s and CHHA’s, ensure timely submission of the Oasis/visit notes, coordinate lab/pharmacy/meds/staff/MD/track/obtain authorization for HMO, coordinate DME orders and supplies, ensure a CHHA plan is present if applicable, inform visiting staff when patient is transferred to Hosp/SNF and after ROC.
REQUIREMENTS:
The Candidate selected for this position must speak and write clearly in English being bilingual is always a plus, be comfortable to communicate quickly and efficiently via email, and text.
