Overview:

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will have experience in pediatrics, diagnostic evaluation, aseptic technique, catheterization, basic life support, eClinicalWorks, behavioral health, dialysis, acute pain management, and inhalation therapies.

Duties:

- Conduct comprehensive assessments of patients' needs
- Develop individualized care plans and goals
- Coordinate and monitor patient care services
- Educate patients and families on treatment plans and available resources
- Collaborate with healthcare providers to ensure optimal patient outcomes
- Maintain accurate and up-to-date patient records
- Provide crisis intervention as needed

Skills:

- Proficiency in pediatric care and diagnostic evaluation
- Knowledge of aseptic techniques and catheterization procedures
- Certification in basic life support (BLS)
- Experience with eClinicalWorks or similar electronic health record systems
- Understanding of behavioral health principles
- Ability to assist with dialysis procedures
- Familiarity with acute pain management protocols
- Competence in administering inhalation therapies
