The Executive Vice President of Care Coordination is a dynamic and visionary nursing leader to provide strategic leadership and direction for Care Coordination services. This leader will be responsible for overseeing all aspects of nursing practice, ensuring the highest standards for patients within Care Coordination, fostering a culture of excellence, and collaborating with other members of the executive team to achieve organizational goals. Leader is charged with assuring the effective, efficient utilization of health care services while developing and implementing patient care standards, processes, and protocols to coordinate safe and efficient transition of patients through the continuum of care. The successful candidate will possess strong clinical expertise, leadership skills, and a commitment to advancing healthcare excellence. The candidate will also have experience in scaling clinical operations for rapid growth.

ROLES & RESPONSIBILITIES:

  • Lead, mentor, and support nursing staff, fostering a positive work environment that promotes professional growth, teamwork, and collaboration.
  • Manage the full cycle of recruitment and selection processes, including job posting, candidate screening, interview and hiring.
  • Assess staffing needs, develop job descriptions, and facilitate workforce planning.
  • Coordinate associate onboarding and orientation programs to ensure a smooth transition for new hires.
  • Oversee associate relations activities, including conflict resolution, disciplinary actions, and grievance procedures, ensuring compliance with legal requirements and organizational policies.
  • Develop and deliver training and development programs to enhance associate skills, knowledge, and performance.
  • Manage associate performance appraisal process, providing guidance and support to managers and associates.
  • Ensure compliance with federal, state, and local employment laws and regulations, including EEO, ADA, FMLA, and OSHA requirements.
  • Serves as a resource and advisor to associates and managers on HR-related issues and concerns.
  • Monitor and evaluate nursing performance metrics, including quality indicators, patient outcomes, and associate satisfaction, and implement initiatives for continuous improvement.
  • Manage nursing budgets, resources, and staffing levels to achieve operational efficiency while maintaining high-quality care.
  • Management of nursing process with the principles of care management within an interdisciplinary and collaborative program, to effectively implement health promotion and prevention of acute and chronic illnesses.
  • Oversight and assessment of post-acute care to determine appropriate referrals throughout the Continuous Care program, with a focus on quality and performance measures that may require intervention.
  • Establishes and implements pathways and processes based on patient acuity providing access to continuous care post discharge.
  • Establish a strong partnership with client development and sales to support growth.
  • Lead the company's quality improvement and patient safety functions. Including accurate documentation and regulatory reporting; with expertise in quality and patient safety in the ambulatory and virtual setting.
  • Leads teams to investigate adverse events, identify root causes, and implement corrective actions to prevent recurrence.
  • Provide support through committees, task forces, and initiatives related to clinical quality, patient experience, and operational excellence in furtherance of value based purposes of:
    • Coordinating and Managing Care
    • Improving Quality of Care
    • Appropriately reducing the costs to or growth in expenditures of payors without reduction the quality of care
  • Transitioning from health care delivery and payment mechanisms based on the volume of items and services provided to mechanisms based on the quality of care and control of costs of care.

  • Conduct regular staff meetings, trainings to promote staff development, teamwork, and accountability.
  • Oversight of patient appointment scheduling, managing patient flow, and optimizing processes to maximize provider productivity and patient satisfaction.
  • Monitor and analyze performance metrics, such as patient volume, appointment utilization, revenue generation, and patient satisfaction scores, implementing strategies for improvement as needed.
  • Partner with medical staff leaders and department chairs to address clinical issues, enhance physician engagement, promote a collaborative practice environment.
  • Leads initiatives impacting satisfaction with customer service programs and patient self-management.
  • Ability to collaborate and build relationships with medical staff and hospital leadership to obtain desired outcomes.
  • Leverages processes and technology to coordinate with the patient's physician a virtual workforce to deliver high quality, cost effective, patient focused care.
  • Demonstrates knowledge on matters regarding physician practice patterns, level of care, care progression, over and under-utilization of resources, medical necessity, denial management, compliance with governmental and private payer regulations, appropriate physician coding and documentation requirements.
  • Stay informed about emerging trends, technologies, and innovations in nursing practice and healthcare delivery, and integrate them into organizational strategies as appropriate.

QUALIFICATIONS - EDUCATION, WORK EXPERIENCE, CERTIFICATIONS:

  • Proven leadership experience in nursing administration or executive roles, preferably in a hospital or healthcare system.
  • Strong clinical background with a comprehensive understanding of nursing practice standards, patient care processes, and healthcare regulations.
  • Excellent communication, interpersonal, and conflict resolution skills.
  • Ability to collaborate effectively with diverse stakeholders, including physicians, administrators, and external partners.
  • Advanced degree in healthcare administration, business, or related field.
  • Bachelor's degree in nursing from an accredited institution.
  • Hold and maintain an unrestricted Nursing license in the state of VA (Compact State).
  • Minimum of 10 years of progressive leadership experience in nursing or healthcare related field.
  • Demonstrated track record of success in driving quality improvement initiatives, patient safety programs, and clinical excellence.
  • Demonstrated experience in recruitment, associate relations, performance management, and organizational development.
  • Demonstrated experience in program development; particularly new programs that involve multiple stakeholders.
  • Ability to manage remotely a large staff of nurses who conduct patient visits virtually. Capable of deploying evidenced based solutions for virtual care enabled by best practice technology is preferred.
  • Ability to build and maintain effective working relationships with associates at all levels of the organization.
  • Strong understanding of healthcare regulations, standards of practice, and accreditation requirements.
  • Proficiency in Microsoft Office suite and electronic health record (EHR) systems.
  • KNOWLEDGE, SKILLS, AND ABILITIES:
  • Possesses a solid foundation of experience in the areas of quality improvement, and patient safety.
  • Possess a working knowledge of hospital operations, case management, and post-acute care.
  • Demonstrates respect with positive interpersonal skills with patients, clients, the public, managers, and associates maintaining confidentiality of patient care and business matters.
  • Adheres to all hospital professional and performance expectations while complying with all established policies and procedures.
  • Effectively presents information, both formal and informally.

  • Exhibits strong analytical skills with a working knowledge of the Electronic Medical Record (EMR). Strong organizational skills and ability to set priorities, multi-task and demonstrate flexibility within the healthcare environment.
