Description

Position Overview
Assist in the provision of quality Resident care in accordance with established policies and procedures under the supervision of licensed nurses. Perform direct care to Residents as assigned by charge nurse, a member of nursing administration, or facility Executive Director. Report any unusual occurrences to designated charge nurse immediately.

Qualifications


  • Unencumbered, active CNA certification in the state of employment.
  • Possess a sincere desire to work with elders and others with a limited capacity for self-care.
  • Mental and physical capacity to perform the essential job functions with or without reasonable accommodations.
  • Work with and provide care to individuals of a wide range of personality types and maturity levels.

  • Communicate and comprehend orally and in writing to effectively receive and share information as necessary to perform job duties.
  • Transportation accessible to enable flexibility of work schedule.

Principal Duties and Responsibilities

  • Assure Resident safety at all times.
  • Willing to address each Resident by his/her preferred name as well as learn and abide by Residents’ Rights, rules and regulations.
  • Maintain confidentiality of all activities, persons, and records.
  • Report directly to the charge nurse of assigned unit and shift.
  • Deliver direct quality-nursing care to assigned Residents and perform clinical duties in accordance with current standards and within the guidelines of established policy and procedure.
  • Establish and maintain positive rapport with all other members of the health care team, Residents and all visitors.
  • Assist in establishing and maintaining a clean, safe and pleasant working environment.
  • Complete accurate documentation of Resident care expediently. Perform or assist Residents in performing activities of daily living, maintaining good personal hygiene, and maintaining proper and adequate nutritional status.
  • Assist in supervision of Residents in dining room to include feeding, cutting meat, delivering and/or arranging food, etc.
  • Assist with procedures such as colostomy bags, drainage bags, sitz baths, enemas, applies/loosens restraints, perineal care, etc., as directed by charge nurse in accordance with established policies and procedures.
  • Perform restorative and rehabilitative procedures as directed by charge nurse.
  • Ambulate Residents with or without self-help devices.
  • Ensure comfort and safety of Residents, report any accident, incident or unusual occurrence to charge nurse immediately.
  • Verify the identity of the Resident prior to administering any assigned treatment, serving food, etc.
  • Turn Residents confined to bed every two (2) hours and document the same.
  • Keep incontinent Residents clean and repositioned to prevent skin breakdown.
  • Assist Residents to and from bathroom, as well as help walking, exercising and moving in and out of bed.
  • Offer bedpans/urinals/bedside commode as needed.
  • Check and report bowel and bladder movements.
  • Ensure that nurse call system is always within reach of Resident.
  • Respond immediately to all Resident call lights.
  • Report any Resident abuse immediately to the charge nurse, Assistant Director of Nursing, Director of Nursing, or Executive Director.
  • Adhere to infection control policies and procedures.
  • Assure preservation of dignity, individuality and independence, and report any violation of Residents’ Rights to the charge nurse, Assistant Director of Nursing, Director of Nursing, or Executive Director immediately.
  • Resident preferences are followed when providing care and service (i.e. meal times, treatment times, type & time of bathing, sleep & wake times).
  • Maintain Resident privacy, values and right for decision making.
  • Immediately notify supervisor and Executive Director for any injury, illness, or if develop a condition, that might impact the aptitude to perform job functions.

Ancillary Duties and Responsibilities

  • Perform duties in compliance with established policies and procedures.
  • Maintain appropriate rapport with each physician.
  • Maintain cooperative attitude towards co-workers.
  • Establish and maintain good working relationship with all members of healthcare team, Residents and all visitors.
  • Assist in maintaining effective employee health and safety programs.
  • Follow policies and procedures during fire/disaster drills and emergencies.
  • Demonstrate a progressive attitude to changes in healthcare trends.
  • Demonstrate a progressive attitude toward the administration and staff of the facility and or consultant personnel.
  • Assist with procedures such as dressings, binders, and slings.
  • Assist Residents with orientation.
  • In cases of death, assist nurse with post-mortem care as outlined by current policy and procedure.
  • Comply with all federal, state and local regulations.
  • Establish objectives and goals for self.
  • Achieve a minimum of two (2) objectives in twelve months.
  • Remain open to constructive feedback from peers and supervisors.
  • Strive to promote own growth in healthcare field by demonstrating knowledge gained by reading literature, attending workshops and in-service education programs, etc.
  • Willingness to work beyond normal working hours and temporarily, in other positions when necessary.
  • Be expected to attend and participate in continuing education programs as directed.
  • Legally and safely operate automotive equipment.
  • Adhere to and comply with any applicable facility insurance guidelines to enable coverage under the policy.
  • Accept additional assignments within scope of training, education, or experience.

Resident Rights

  • Maintain the confidentiality of all Resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
  • Ensure that the Resident’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are well established and maintained at all times.
  • Review Resident complaints and grievances and make written reports of action taken. Discuss such actions with Resident and family as appropriate.

Safety and Sanitation

  • Ensure that you and all facility personnel, Residents, visitors, etc., follow established safety regulations, to include fire protection/prevention, smoking regulations, infection control, etc.
  • Ensure that the building and grounds are maintained in a clean, safe, and sanitary manner as well as in good working order by reporting all concerns to management.
  • Ensure that you and all facility personnel follow established regulations governing the use of labels and SDSs for hazardous chemicals.
  • Ensure that facility procedure manuals identify safety precautions and equipment to use when performing tasks that could cause bodily injury or exposure to a hazardous chemical or infectious waste.
  • Ensure that you and all facility personnel follow established ergonomics policies and procedures to prevent self-injury.
  • Ensure that you and all facility personnel performing tasks that involve potential exposure to blood, body fluids, or hazardous chemicals are appropriately trained prior to performing such tasks.
