Overview
Our specialty pharmacy program is growing and looking for experienced pharmacists to join our team. This position will mainly support our Oncology patients across the state. Daily responsibilities include clinical review of prescriptions, collaborating with pharmacist and provider team embedded in clinic, conducting patient and provider outreach, and participating in specialty management program. Team members are office based, with the opportunity for partial remote work available. Schedule is Monday – Friday day shift, includes no weekend/evenings/holidays—we are based out of the IU Health Integrated Services Center located in Plainfield, IN.
This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.
• Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.
• Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be required.
