Direct Support Professional Position in Reading, Pennsylvania
Do you enjoy helping others to be independent and active in their home and in their community? Are you looking for up to 18 part-time hours, with the possibility of more hours? Do you enjoy providing individualized, 1:1 direct care? If so, this may be a perfect opportunity for you.
We are seeking qualified candidates for an in-home, Direct Support Care person to work with a mature woman in Reading, PA. She is very smart, plays the organ, is involved with her church, and is most happy when her schedule is maintained. She loves shopping, music, and doing crafts.
In addition to supporting activities of daily life, other supports may include: prompting through tasks, supporting her independence, problem solving, maintaining her safety, community engagement, and some light housekeeping.
Shifts Available:
Currently looking for staff to work 32 hours per week, including Saturday overnights.
Saturdays: Overnight shift (8:00 pm to 8:00 am) (Consistently)
Second shift (start time varies depending upon the day and ends at 8:00 pm)
Occasional weekend shifts may be needed based on your availability.
Your schedule can be discussed if you’re the right fit and interested.
Additional hours may be available based on your availability.

Pay:
Habilitation: $20.00-$21.50 per hour depending on experience
Companion: $16.00-$17.50 per hour depending on experience

Experienced staff will:
*Be responsible for the supervision, safety, and welfare of the individual at all times.
*Be responsible for establishing and maintaining a productive daily activity pattern by consistently facilitating the individual’s individual support plan (ISP).
*-Provide skills training, assistance and/or supervision to the individual in the areas of daily living
*Assist with prompting/reminders throughout day-to-day activities within the individual’s home and in the community
*Serve as a member of the team for the development of ISP. Gives feedback on the individual's performance in accomplishing goals and objectives of the ISP.*
*Coordinate, organize and/or assist with household activities per individual’s ISP goals.
*Communicate information consistently and will be responsible for documenting incidents on appropriate forms and verbally relaying information to the Managing Employer.
*Demonstrate basic knowledge to assure a safe and healthy environment.
*Demonstrate recognition of, and properly report suspected abuse and neglect.
You MUST…
· Have at least 6 months experience in the human service field working with individuals with intellectual and physical disabilities
· Be 21 years of age or older
· Have a valid driver’s license, access to a reliable vehicle, clean driving record, and be willing to transport individual in the community
