The Clinical Nurse Liaison will ideally have sufficient experience in infusion therapy, central venous access, case management, home health or hospice experience, and possess exceptional accountability and ownership traits.

What You'll Be Doing

As a registered nurse for Intramed Plus servicing the Florence and Pee Dee Region of South Carolina, you are the direct liaison and point of contact for coordinating and collaborating with multi-disciplinary teams to ensure proper patient treatment and care. The Clinical Nurse Liaison will assist with the delivery of quality patient care services, on-site referral coordination, clinical follow-ups, patient education and training, and assist in transitional care coordination from the hospital to the home.

What We're Looking For

• Registered nurse with a current license in the state of South Carolina
• 2 years of nursing experience

• Experience in infusion therapy, central venous access, case management and home health/hospice is preferred

• Established relationships with hospital case management departments
• Ability to communicate at various levels effectively and proactively throughout internal and external organizations
• Excellent communication and interpersonal skills
• Team player with the ability to partner across the company to drive success for everyone
• Exceptional organization skills and the ability to multitask
• Tech savvy, proficient in the use of modern technology and the Microsoft Office Suite

Work Schedule

This full-time position typically works eight-hour shifts, Monday through Friday, and is primarily based at MUSC Florence. The ideal candidate must be available to assist with on-call responsibilities - this may include weekends and/or holidays.

What We Offer
