Under the direction of the HAWP Clinical Services Manager and HAWP Medical Director within the Program of All Inclusive Care (PACE) the Registered Nurse - Home Care will assist in the provisions of home care and home health nursing care including but not limited to; in person assessments, health
education, health promotion, nursing treatments, family support, medication management, wound care and other standard nursing duties. The RN-HC will help formulate nursing care plans and serve as a point of contact for patients, families, and caregiver related entities to oversee and coordinate functions of in-home
clinical care and caregiver education. They will act as a liaison between patients in the community and, their caregiver other PACE clinicians as an interdisciplinary team member (IDT). The RN-HC engages in face-to-face, telephonic, and in-home visits and supports care coordination and clinical care services in the
home as needed.
Registered Nurse (RN) Pay Range: $42.50 - $62.22
Education – Graduate of a professional nursing school. Bachelor's Degree in Nursing preferred.
Experience – At least one (1) years of experience working with individuals with a chronic condition or disability, or working in an inpatient or outpatient primary care setting. One (1) or more years (s)’ experience working in a home health setting preferred. Asian Pacific Islander language proficiency preferred but not required.
Other Requirement(s) – Current WA State RN License, BLS Certification Required. Must be willing to
travel within PACE service area for visits with patients in the community.
