We offer
The Home Care Access Liaisoncoordinates and facilitates the non-clinical activities of in-home participantcare by acting as a liaison between the PACE Organization and ContractedAgencies and IP/Caregivers to enable them to focus on quality care andparticipant satisfaction. Trains agency staff and IPs on the PACE program andensures compliance with CMS regulations related to Personnel and ServiceProvision. The Home Care Access Liaison coordinates and maintains IP andoutsourced agency caregiver schedules and medical records in collaboration withthe Director of Clinical Services and HAWP Nurse; maintains participantconfidentiality; maintains a hazard-free work environment. As appropriate,responds to participant requests and complaints; escalates requests/complaintsto the appropriate staff as necessary. Works closely with participants,families, visitors, providers and staff to identify needs, answer questions,resolve problems, provide information and follow up. Responsible forcoordinating and implementing Plans of Care and SDRs and Grievances accordingto the appropriate workflows. This position is amember of the Interdisciplinary Team, pursuant to 42 CFR §§ 460.102(b) and (d),as the Home Care Coordinator role.Hourly pay range: $26.85 - $39.31
Education - High school diploma or equivalent.
Experience - Experience working with the elderly, home careagencies, home health agencies or PACE preferred.Other Requirement(s) - Valid WA State Driver’sLicense and proof of insurance. Must have active CPR and First AidCertification, or be willing to obtainit within 1 month of hire.

For over 40 years, ICHS has provided culturally-appropriate and multilingual care to improve the health of our community. ICHS began in 1973 as a small storefront clinic in Seattles International District.
