CHOICES Lead Direct Support Professional **This position is eligible for a $1,500 hiring bonus Department: CHOICES Job Status: Full Time FLSA Status: Non-exempt Reports To: Residential Supervisor Position Supervises : NA Work Schedule: Full-time position requiring flexible schedule POSITION SUMMARY Assist the residential Supervisor in ensuring quality of services to people supported ESSENTIAL FUNCTIONS STATEMENT(S) Individual Services
Track, document, schedule and transport individuals to medical appointments.
Ensure medical needs are reported and documented.
Complete med chart checks ensuring all meds are current.
Assist in following NSLP guidelines.
Documentation and Communication
Ensure data is documented daily.
Submit monthly reports, not limited to:
Approved Contacts
Supply orders
Fire/Severe Weather
Department of Transportation Reporting
National School Lunch Program
Maintenance Report
Facility
Maintain a high standard of cleanliness in facility and report all maintenance issues.
Maintain safe working conditions.
Ensure vehicle safety and maintenance.
Other
Provide safety-sensitive transportation services as needed.
Responsible for other duties as assigned.
POSITIONS QUALIFICATIONS SKILLS & ABILITIES Education: Bachelor's Degree preferred. Experience: Six months of residential experience or an equivalent combination of education/experience. Skills: Excellent communication skills and time management are necessary. Certifications & Licenses: Valid driver's license. Other Requirements: Requires consistent exercise of discretion and judgment in the performance of duties. PHYSICAL DEMANDS A. Ability to stand, walk, sit, manually manipulate, reach outward and above shoulders, speak, climb, crawl, squat, kneel, and bend on a daily basis. B. Capability to lift, carry, push or pull up to 100lbs on a daily basis.
