JOB SUMMARY:
This is an exciting new treatment team to ensure that Whole Health Care is being provided in these existing divisions. Working collaboratively with the residential and housing teams, Health Connect with fill in any gaps around physical care. A registered professional nurse will be responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as case finding, health education/teaching, and health counseling; nurses provide interventions supportive to or restorative of life and well-being, and execute regimens prescribed by a physician, dentist or other licensed health care provider. The nurse is responsible for coordinating and overseeing all health education, required by people living with us. The incumbent of this position will be a registered professional nurse (RN) with either an associate degree or a bachelor’s degree, a current New York State license, and experience working with adults with serious mental illness.
ESSENTIAL TASKS:

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
License and current registration to practice as a registered professional nurse in New York State; prefer at least one year post RN licensure clinical nursing experience working with individuals with mental disabilities.
