The Health Navigator focuses on the prevention and early intervention as it relates to the Behavioral and whole health continuum of care. The Health Navigator is responsible for Member outreach and navigation support on issues of access and use of IEHP services and programs. Under the direction of the Manager of Community Behavioral Health & Social Supports and the Supervisor for the Health Navigator Program, the Health Navigator is proactive and responsive to Members’ needs in a friendly and professional manner. The Health Navigator helps individuals overcome barriers and engage in equitable and quality care. This position provides training and will assist in program development for Members in working with community leaders on different levels.

Key Responsibilities:

1. Screen and evaluate Members; Identify physical, behavioral health, and social care gaps and support direct linkage to treatment and services. 2. Provide outreach and education about IEHP health benefits, care teams, and community resources, coordinating care as appropriate and necessary. 3. Work with IEHP health plan Members & community partners, providing in person interactions in their homes or community location. In person interactions include, but are not limited to, education facilitation, training facilitation, and presentation facilitation. 4. Provide comprehensive whole person care, addressing family systems needs as well as individual Member needs.5. Assist in the development and facilitating of trainings based on community partner needs such as Applied Suicide Intervention Skills, IEHP benefits, and motivational interviewing.6. Responsible for building capacity by tapping into existing partnerships both internally/externally within the community creating sustainable best practices to support Member needs. 7. Provide education on preventative measures related to health behaviors and identify protective and risk factors impacting Members. 8. Liaise between the Member, IEHP, and network providers to assist Members with appropriate and timely access to services.9. Present cases to the CBH leadership and team, collaborating with internal departments as necessary and appropriate. 10. Perform necessary screenings and assessments (PHQ-9, SDOH, and preventative measures) with each Member, engaging in appropriate follow up and on-going care coordination. Utilize evaluation tools, pre and post assessments and available data to develop individualized plans of care for assigned Members and families.11. Provide advocacy for Members on issues of access and use of primary care and prevention services, along with conducting outreach activities, as needed in addressing program goals and objectives in physical, behavioral, and social needs.12. Build and maintain relationships with organizations, providing partnership with providing operational & technical support when necessary.13. Identify key external stakeholders and opportunities to increase collaboration to produce, perform, and/or deploy needed services for improvement within each partnership.14. Organize and plan an actionable plan to engage with assigned schools or organizations. 15. Attend required trainings enhancing skill level and education pertaining to the following topics that include but are not limited to motivational interviewing, health benefits, targeted chronic illnesses, suicide prevention, youth mental health and first aid, engaging schools and more. 16. Maintain an understanding of Department of Health Care Services and California Department of Education policies pertaining to the following topics that include but are not limited to, community schools, CYBHI, SBHIP, and any APL that aligns with school strategy. 17. Complete timely and accurate documentation and requested record keeping.

Qualifications

Education & Experience

  • Bachelor’s degree from an accredited institution required.
    • In lieu of the required degree, a minimum of four (4) years of additional relevant work experience is required for this position. This experience is in addition to the minimum years listed in the Experience Requirements above.
  • Four (4) years of experience in any/all of the following:
    • Promotora/Community Health Worker experience
    • Community outreach
    • Community event participation
    • Customer service experience in a related field such as public health, CHW work, teaching/education, or Case coordination
    • Delivering presentations/trainings to varied audiences
    • Managed care experience
  • Bilingual required*

Key Qualifications

  • Must have a valid California Driver’s license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points. 
  • Be a proactive team player who is also able to work independently in assigned communities, and build rapport with diverse Members, providers, and local partner agencies.
  • Knowledge of Medi-Cal/managed care environment required. Understanding of and sensitivity to multi-cultural communities
  • Proven ability to:
    • Demonstrate mature judgment and initiative.
    • Maintain Member confidentiality.
    • Have a high degree of patience.

Pay Range

USD $27.43 - USD $35.66 /Hr.
