The Health Navigator focuses on the prevention and early intervention as it relates to the Behavioral and whole health continuum of care. The Health Navigator is responsible for Member outreach and navigation support on issues of access and use of IEHP services and programs. Under the direction of the Manager of Community Behavioral Health & Social Supports and the Supervisor for the Health Navigator Program, the Health Navigator is proactive and responsive to Members needs in a friendly and professional manner. The Health Navigator helps individuals overcome barriers and engage in equitable and quality care. This position provides training and will assist in program development for Members in working with community leaders on different levels.
