Title: RN Home Health Case Manager

Location: Green Bay, WI 54311

Contract Duration: 3 Months Contract (Possibility of Extension)

Shift Requirements: Day Shift (5 Days a Week 40/hr a Week 8/hr a Day with alternate weekends)

Duties:

Wound care, including wound vacs, pleural drains, ostomy care; peripheral blood draws, central line care, IV administration through multiple devices including elastomeric and IV pumps, excellent assessment skills, experience with computer documentation

Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.

1-year of Home Health Case Management experience and have BLS and WI RN license (or compact) in hand.

Experience & Education:

This position will manage 20-30 patients per day.

Coverage area: Green Bay Market, Brown County, Parts of Outagamie

Job Type: Contract

Pay: $80.00 - $82.00 per hour

Expected hours: 40 per week
