FULL-TIME POSITIONS ARE AVAILABLE
Travel Reimbursement (Tolls/After 45 miles, 0.66 per mile paid)
Potential Paid Drive (dependent on facility)
Professional Development Reimbursement (License/Certification Fees Paid)
FULL-TIME EMPLOYEES ONLY (35-40 hours): Benefits/401K/PTO Accrual/Paid Vacation/Paid Holidays
Essential Job Functions:
·Provide clinical nutrition and dietary management support.
·Develop monthly quality assurance reports.
·Lead nutrition-at-risk (NAR) meetings and provide clinical nutrition interventions and care plans.
·Complete monthly quality assurance audits to ensure regulatory compliance for state/federal survey preparedness.
·Participate as an essential Interdisciplinary Team Leader within each account.
·Communicate with facility representatives for coordination of care and services.
·Complete nutrition assessments and MDS sections
·Participate in weekly team calls and complete QA data.
MINIMUM QUALIFICATIONS:
·A minimum Bachelor of Science degree completed.
·Registered Dietitian as per Commission on Dietetic Registration
·Registered Dietitian State Licensure
·Valid Driver’s License
·Electronic medical record (EMR) experience (PCC/Matrix) is preferred but not required.
·Experience in LTC and managing multiple LTC buildings is a plus but not required.
