Job Description:

Licensed Home Care Agency is seeking Registered Nurses to fulfill Per Diem Positions.

Positions - Per Diem Registered Visiting Nurse

Job Duties include:

Perform Health Assessments for patients in their home to assess a patient’s health care status and needs at start of care and then every 6 months and as necessary. Initiate Plan-of-Care and make necessary changes, as necessary. Supervise paraprofessional staff on patient plan of care

Duties and Responsibilities:

• Maintain close communication with the Director of Patient Services and interdisciplinary team
• Responds to phone calls and emails in a timely manner
• Reporting all issues and concerns to the Director of Patient Services immediately
• Completes documentation accurately and submits timely to agency
• Provides the highest quality care in accordance with DOH regulations

Qualifications:

• Graduate of approved School of Nursing
• Current NYS RN License
• Able to function independently with minimum supervision
• Demonstrates strong critical thinking skills
• Ability to travel to patient’s homes

Experience:

• Home Care: 1 year (Required)

Pay: $75.00 - $100.00 per hour
