Independent Adult Day Centers is Hiring a Clinical Liaison Near Indianapolis, IN

Position: Clinical Liaison

Reports To: VP of Business Development

JOB SUMMARY:

The Clinical Liaison (“CL”) manages relationships with outside medical providers. The CL educates case managers, discharge planners, social workers, physicians and other medical professionals about the IADC/IHHC continuum of care in order to generate referrals for both Adult Day and Home Health. The CL performs initial patient assessments, facilitates service line follow up steps, and ensures completion of necessary documentation in compliance with Company policy and regulatory requirements for home health admissions. This position requires high self-supervision and professionalism, superior communication and interpersonal skills, and commitment to providing high quality care. The CL must possess a high degree of clinical fluency.

ESSENTIAL FUNCTIONS:

· Develop and maintain relationships with key leadership and discharge planning professionals at hospitals and inpatient rehabilitation hospitals.

· Provide ongoing updates on home health care, adult day services, attendant care, health care coordination and other services provided by IADC.

· Provide exemplary customer service to referral sources, patients, and patients’ family at all times.

· Monitor and manage hospital referral systems for new IADC referrals and for existing IADC Guest and patient updates.

· Make regular, planned calls to visit inpatient Guests of IADC, secure new referrals, build census, and meet established goals on new and existing referring physicians; facilities; case managers, discharge planners, and social workers in hospitals and other health care providers.

· Conduct pre-discharge facility visits to educate the family and determine the need/eligibility for IADC services.

· Ensure effective communication and collaboration with Home Health staff. Actively participate in regular agency meetings, bringing relative data, reports for review, and information about changes within accounts and referral sources.

· Forward referrals for Adult Day, Attendant Care, and Health Care Coordination to the relevant service area Director.

· Assist Home Health Agency in timely processing of physician orders.

· For all new Home Health Patients, inform hospital personnel, patient and/or patient family of case acceptance and document all demographic, clinical, payer and other information as required to support diagnosis and home healthcare orders. Inform the Home Health clinical staff of patient’s acceptance and ensure that patient’s needs are appropriately matched.

· Abide by all operational compliance with internal controls and reporting.

· Provide education and/or continuing education programs for referral sources within guidelines of company policy. Utilize appropriate company resources to maintain and develop business opportunities. Maintain market awareness and prepare competitive updates.

· Work with Home Office Leadership, Home Health Administrator, and service area Directors to prioritize accounts in accordance with the market/territory sales plans ensuring effective maintenance, business development and territory management of assigned accounts.

· Participate in Quality Assurance Performance Improvement projects.

· Timely and accurately document notes in CRM.

· Update CRM with referral source and industry contact information and obtain new referral source and industry contact information.

QUALIFICATIONS:

· Education: College degree required. Associate’s degree with clinical licensure or Bachelor’s degree in a related field preferred.

  • Professional Experience:
  • One year healthcare experience
  • Two years' experience in business development, relationship management, clinical liaison or marketing in home health, hospice, or physical therapy
  • Other requirements:
  • Existing relationships and lead generation history with Indianapolis area hospitals, skilled nursing facilities and physician’s offices
  • All current vaccinations as required for entrance to hospitals and other healthcare facilities

ESSENTIAL PHYSICAL REQUIREMENTS:

The employee will be required to attend meetings in the Company office, Adult Day Centers, regularly visit the facilities of healthcare providers and may also visit patient homes. A significant amount of driving is required. A significant amount of standing, walking lifting and/or bending is required. While performing the duties of this job, the employee is also regularly required to sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move objects with or without assistance. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee must be able to use general office equipment such as computers, photocopiers, telephones, etc. The employee must be able to properly use/handle medical equipment and supplies.
