 The responsibilities of the Clinical Liaison include: 

  1. The Clinical Liaison will be fully knowledgeable of all Hooverwood’s Living services Lines.
  2. Provide information, tours and interviews for residents, responsible parties and / or family members who have expressed interest in any of the Hooverwood services lines.
  3. Provide pre-admission assessments of applicants to determine the need for facility placement. Assessments may require visits to area hospitals, assisted living, retirement communities, other nursing homes, and/or the resident’s residence as needed. Work in conjunction with Administration, Nursing, Business Office, etc. when making admission related approvals and when assuring compliance with CICOA, PAS, insurance company, Medicare, Medicaid, private pay, etc., regulations and requirements. 
  4. Assure all admission documents such as: financial, medical records, physician orders, resident notices of rights, and HIPAA policies are signed by the resident and / or resident responsible party. Assure that completed admission contracts and all other documents are turned into the Administrator for review and signature in a timely fashion.
  5. Assure that all P.O.A (Power of Attorney) information is entered into PCC (Point Click Care) in an accurate and timely measure. 
  6. Assure new residents and family members are well introduced to their new environment immediately upon arrival, while specifying specific needs to the Social Service staff.
  7. Act as an advocate for the facility at all times. Assist Hooverwood Administration as appropriate to investigate and respond to legitimate complaints a resident, resident family or resident legal representative may have.
  8. Assure that complete and accurate information is provided to the Receptionist for the completion of admission face sheets, daily census, etc.
  9. Assure that all admission-related documents, brochures, contracts, etc., are accurate, timely, and fully stocked.
  10. The Clinical Liaison builds and maintains relationships with Discharge Planners, Case Managers, Physicians, and other potential referral sources. And follow-ups with referral sources with case studies and updates.
  11. Attend and provide input, direction, and feedback at the following meetings: Team meetings, Quality Assurance Committee, Admissions Committee, Rehabilitation Committee, Marketing Committee, Eldersource Case Management Committee, NORC Advisory Committee, or any other Board or staff committees as may be assigned.
  12. Participate in the planning, budgeting, and budget control of the Admissions & Marketing Department’s annual operating budget.
  13. Maintain admission related logs for Board Meeting, marketing, productivity, and utilization review purposes.
  14. The Clinical Liaison should have thorough knowledge of the general market, specific product, services, and acuity trends within the market. 
  15. The Clinical Liaison shall fully comply with all aspects of Hooverwood’s Corporate Compliance program and its Code of Conduct. 
  16. The Clinical Liaison shall perform any other duties, as may be assigned by the Administrator.

Requirements

The Clinical Liaison:

  • Must possess and maintain an LPN/R.N. degree from an accredited School of Nursing
  • Must maintain a valid LPN/ R.N. license from the State of Indiana
  • Experience with a geriatric population in a long-term care facility. 
  • Excellent assessment skills from visual, written, verbal, and electronic systems. 
  • Effective communication skills, i.e. orally and verbally. 
  • Self -motivated and goal oriented. 
  • Must be able to work Monday – Friday (on-call rotation every other weekend). 
  • Must have a valid Indiana Driver’s License.
