Job Overview:

We are seeking a dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide compassionate care to patients in the comfort of their own homes. This position offers the opportunity to make a meaningful impact on patients' lives through personalized healthcare services.

Responsibilities:

- Conduct patient assessments and develop individualized care plans

- Administer medications and treatments as prescribed by physicians

- Answer all intake referrals and staff patient accordantly

- Monitor patients' conditions and report changes to the healthcare team

- Educate patients and their families on managing illnesses and injuries

- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care

- Perform wound care, IV therapy, and other specialized nursing procedures as needed

Requirements:

- Valid Registered Nurse (RN) license in the state of practice

- Knowledge of medical terminology and patient care best practices

- Experience in behavioral health, discharge planning, and utilization management is preferred

- Strong understanding of physiology and medical records management

- Background in emergency room (ER) or acute care settings is advantageous

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a multidisciplinary team
