PEER SUPPORT SPECIALIST– MOBILE CRISIS TEAM

General Responsibilities

The Peer Support Specialist will provide mobile crisis, peer mentoring, and case management services to clients and their families. The Peer Support Specialist will inspire and strengthen the voice of the patient. They strive to make a difference through education, empowerment, and support. They will advocate for the patient with other systems.
The Mobile Crisis program specializes in providing services to patients, families, and other community members who have an urgent mental health need. The mobile crisis team will respond to all approved referrals. During response times, a team consisting of two members will respond. The mobile crisis team will assess the situation and create a safety plan for the individual and/or family. This may include providing multiple services until transitioned to an appropriate level of care.
Qualifications
Bachelor or Associate degree in Social Work, Psychology, or other mental health related field preferred; High School Diploma with experience will be considered in lieu of a degree. Must have lived experience as a current or former recipient of services; Certified Recovery Specialist or willing to receive CRS training required.

1. Direct Service Activities
  • Implements activities for patients referred to mobile crisis.
  • Assists in the Assessment of the crisis situation.
  • Assists in the creation of a safety plan with the patient and, if applicable, includes family and/or natural supports.
  • Models appropriate problem-solving and conflict-resolution techniques.
  • Models cooperative relationships with other community providers involved.
  • Fosters an environment in which clients and their families are supported in achieving stabilization goals.
  • Consults with on-call therapist and on-call psychiatrist, as appropriate.
  • Provides therapeutic services for clients, including family and natural supports, involved in mobile crisis services including home based services needed to maintain safety and stability.
  • Monitors clients current functioning and assesses risk
  • Coordinates communication and services provided by other CMHC programs and appropriate community agencies.
  • Assists client/family in obtaining necessary resources including obtaining presumptive eligibility and Medicaid as appropriate.
  • Advocates for patient needs (including needs within family) within the mental health system and community.
  • Provides transportation to treatment activities and appointments, when necessary.
  • Teaches and models appropriate skills to patient and family to encourage health family communication and roles and establish safety and stability.
C. Provides emergency services phone coverage as available.
2. Client Care Hours
A. Provides client care hours on an as needed basis including follow-up care and assistance in transitioning to on-going care.
3. Community Activity Contacts
  • Actively monitors and encourages appropriate use of community resources to assist clients in attaining maximum self-growth.
  • Seeks out health care, social services, employment and/or vocational development resources for clients.
4. Record Keeping Activities
  • Complies with timeliness of record keeping in accordance with agency policies and procedures; i.e. service notes submitted following established policies and procedures, time deadlines in the Outpatient Assessment and Treatment Plan Policy are met, etc. so that all records are submitted within the time frame allowed.
  • Completes all documentation needed to ensure fair and accurate data collection for billing, fee setting, management information system functions, and other purposes in accordance with agency policies and procedures.
  • Completes attendance records, travel vouchers, PTO requests, staff development requests, employee self-evaluations, etc. accurately and efficiently in accordance with agency policies and procedures.
5. Staff Development Activities
  • Actively participates in the Center’s Staff Development Programs by fulfilling identified goals.
  • Participates in multi-disciplinary staffing, team meetings, full-staff meetings, etc., as appropriate to position.
  • Serves on Performance Measurement and other committees as assigned.
  • Completes Bloodborne Pathogen/TB/Infection Control/Risk Management training at employment and annually thereafter.
  • Completes MI, CRS, and QPR training.
  • Attends conferences, workshops, meetings, in-services, etc. to enhance professional knowledge and development according to the need identified in the IEDP.
  • Demonstrates active involvement in supervision as evidenced by constructive participation, initiative, and apparent effort.
6. Other Duties
  • Performs other duties as assigned.

  • Immediate Supervisor: Coordinator of Emergency Services
  • Supervises: No direct supervisory responsibilities

Knowledge and Skill Required
  • Ability to work effectively with clients of all ages and their families and natural supports.
  • Basic knowledge of needs of patients with SED, SMI, Addictions, and Dual Diagnosis.
  • Demonstrates good judgment, time management, and record keeping skills.
  • Good written and verbal communication and assessment skills.
  • Ability to work energetically and independently.
  • Ability to work with the public and community agencies.
  • Capacity to access other resources and services for rural client population.
  • Ability to work within a treatment team setting.
Special Requirements for Position
  • Willingness to travel and work in a client’s home or other community setting.
  • A valid driver’s license with a good driving record.
  • Willingness to travel on behalf of agency business.
This position will be eligible for shift differential.
