JOB DESCRIPTION
Reports to: Director of CCBHC Project for Administrative. Dr. Bakhtier for clinical support.
Summary/objective (brief summary and overall objectives of the job):
The nurse will provide various nursing services to the clients of the Crisis Stabilization and Rehabilitation Services including triage assessment, nursing assessment, first aid, point of care tests (such as urine drug screens, pregnancy tests, and covid tests), and referrals for a full primary care assessment or to emergency care as appropriate. The nurse will participate in treatment planning, transition, and discharge planning. The nurse will schedule appointments with primary care and psychiatry as appropriate or secure same day assessments as needed. The nurse will advocate for the client with other systems.
Essential functions (functions and how an individual is to perform them, and the frequency with which the tasks are performed; the tasks must be part of the job function and truly necessary or required to perform the job):
  • Provide triage assessment of client’s medical all associated reports and paperwork.
  • Provide a full nursing assessment and make appropriate referrals.
  • Provide first aid.
  • Provide point of care tests including urine drug screens, pregnancy tests, and covid tests.
  • Draw blood for lab work or collect other samples and package samples.
  • Participate in treatment team meetings for safety/crisis planning, transition/discharge planning.
  • Coordinate care with primary care provider and psychiatric provider as needed.
  • Follow and oversee infection control procedures.
  • Complete all agency documentation in a timely manner.
Competency (knowledge, skills and abilities):
  • Knowledge of and experience with crisis de-escalation, crisis/safety planning, and first aid.
  • Knowledge of triage and nursing assessment.
  • Knowledge of integrated care.
  • Knowledge of social determinants of health.
  • Skills in advocacy, organization, and communication.
  • Skills in blood draws, point of care testing, and other lab collection, storage, and shipping.
Supervisory responsibilities (direct reports, if any, and the level of supervision): This position will supervise the peer specialist, milieu specialist, and behavioral health technician from their shift.
Work environment (the work environment; temperature, noise level, inside or outside, or other factors that will affect the person's working conditions while performing the job): Crisis stabilization setting with room temperature and primarily inside an office.
Physical demands (the physical demands of the job, including bending, sitting, lifting and driving):
While performing the duties of this job, the employee is regularly required to sit, stand, walk, bend and lift objects of up to 25 pounds.
Position type and expected hours of work (full time or part time, typical work hours and shifts, days of week, and whether overtime is expected):
This position will work full-time, 40 hours a week, Monday through Friday.
1st shift- 8:00 am-4:30 pm
2nd shift- 4:00 pm-12:30 am
3rd shift- 12:00 am-8:30 am
Or, this position will work part-time, 24 hours a week, Saturday through Sunday.
12:00 am- 12:30pm both days
or
12:00 pm- 12:30 am both days
Travel (percentage of travel time expected for the position, where the travel occurs, such as locally or in specific countries or states, and whether the travel is overnight):
Local travel will be required on an as needed basis.
Required education and experience (education and experience based on requirements that are job-related and consistent with business necessity):
A degree in nursing with a license as RN.
Preferred education and experience (preferred education and experience based on requirements that are job-related and consistent with business necessity):
Bachelor’s degree RN preferred. Two years of experience preferred.
Additional eligibility qualifications (additional requirements such as certifications, industry-specific experience and the experience working with certain equipment):
  • ASIST Training
  • CRS training/certification.
  • Handle with Care training
  • QPR/Mental Health First Aid.
  • Trauma Informed Care Training
  • Point of Care Testing
  • Lab specimen collection including ability to draw blood.
