Registered Nurse Case Manager

Job Summary:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and managing the care of patients, ensuring they receive the highest quality of healthcare services. Your role will involve patient assessment, care planning, and collaboration with other healthcare professionals to provide comprehensive care.

Responsibilities:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Monitor and evaluate patient progress, making necessary adjustments to the care plan as needed
- Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated and effective care
- Educate patients and their families on their medical conditions, treatment options, and self-care techniques
- Administer medications and treatments as prescribed by physicians
- Document patient information accurately and maintain confidentiality in accordance with HIPAA regulations
- Provide emotional support to patients and their families throughout the healthcare journey
- Advocate for patients' rights and ensure they receive appropriate medical services
- Participate in case conferences and interdisciplinary team meetings to discuss patient progress and develop care strategies

Requirements:
- Valid Registered Nurse (RN) license in the state of Ohio with no board action
- Minimum 1 years of experience in acute care or managed care setting
- Strong knowledge of medical terminology, physiology, and healthcare practices
- Proficient in conducting patient assessments and interpreting vital signs
- Familiarity with HIPAA regulations and ability to maintain patient confidentiality
- Excellent communication skills, both verbal and written
- Ability to work collaboratively in a multidisciplinary team environment
- Strong organizational skills with attention to detail
- Compassionate nature with a commitment to providing exceptional patient service
