PRIMARY CARE NURSE PRACTITIONER
Fresno, CA
Salary Range: $145,831.32 - $169,274.69 per annum
Under the guidance of the Medical Director, the Primary Care Nurse Practitioner collaborates with Physicians and the interdisciplinary team to provide assessments, develop care plans, deliver primary care, and offer continuing medical education to patients.
Key Job Responsibilities:
- Conduct thorough in-person assessments, including initial, scheduled, and unscheduled assessments, and ensure continuous medical coverage for patients
- Cooperate with physicians and the interdisciplinary team to develop individualized care plans for each patient
- Participate in discharge planning from acute or long-term care facilities along with physicians and the interdisciplinary team
- Regularly visit and provide nursing care to patients in nursing homes based on their needs and requirements
- Collaborate with physicians, community-based PACE physicians, and the interdisciplinary team to oversee and ensure effective care coordination
- Review outcomes and utilization data, such as hospitalizations and specialist usage, in consultation with the Medical Director to establish best practices and policy enhancements
- Participate in on-call rotations during nights and weekends
Job Requirements:
- Graduation from an Accredited School of Nursing
- Certification as a Nurse Practitioner with Prescriptive Authority
- Valid RN State License
- Minimum of two years of experience working with elderly individuals
- Up-to-date knowledge of chronic care and geriatric-related issues, as well as best practices
- Valid State Driver’s License with a clean DMV record
- Submission of recent Vehicle Insurance copy is mandatory

- Certification as a Nurse Practitioner with Prescriptive Authority
- Licensure in the state of California
- Experience in geriatrics (preferred)
