Duties & Responsibilities:
Ensures compliance with documentation standards, program and agency policies within assigned work unit consistent with the agency’s mission and vision.
Reviews family service coordinators’ documentation and reports to ensure that the case management needs of the enrolled children and their families are adequately addressed.
Enhance the work unit’s capacity to provide a high standard of practice, by fostering good working relationships and providing consultation, technical assistance and case management support to Family Service Coordinators, including coverage during FSC absence.
Provides feedback to the Program Director in regards to the case management needs of the program and some potential means to achieve the overall case management expectations of the agency.
Ensures that all incidents are reported according to the agency policy and procedures.
Provides leadership in the development and implementation of case management best practices and trainings. Collaborate with other staff as appropriate to implement effective and efficient case management practices agency wide.
Prepare reports and maintain data relative to the responsibilities and performance of Family Service Coordinators.
Work as a liaison between ICAN staff and the Case Management staff at the Department of Social Services.
Education/Experience:
Bachelor's degree (B.A.) from an accredited four-year college or university, a major in social work or a related field is preferred.
A minimum of two years of experience providing service coordination for community-based services to individuals, preferably children, with special needs.
At least 1-2 years of supervisory experience is preferred.
