Duties & Responsibilities:

• Ensures compliance with documentation standards, program and agency policies within assigned work unit consistent with the agency’s mission and vision.

• Reviews family service coordinators’ documentation and reports to ensure that the case management needs of the enrolled children and their families are adequately addressed.

• Enhance the work unit’s capacity to provide a high standard of practice, by fostering good working relationships and providing consultation, technical assistance and case management support to Family Service Coordinators, including coverage during FSC absence.

• Provides feedback to the Program Director in regards to the case management needs of the program and some potential means to achieve the overall case management expectations of the agency.

• Ensures that all incidents are reported according to the agency policy and procedures.

• Provides leadership in the development and implementation of case management best practices and trainings. Collaborate with other staff as appropriate to implement effective and efficient case management practices agency wide.

• Prepare reports and maintain data relative to the responsibilities and performance of Family Service Coordinators.

• Work as a liaison between ICAN staff and the Case Management staff at the Department of Social Services.

Education/Experience:

• Bachelor's degree (B.A.) from an accredited four-year college or university, a major in social work or a related field is preferred.

• A minimum of two years of experience providing service coordination for community-based services to individuals, preferably children, with special needs. 

• At least 1-2 years of supervisory experience is preferred.
