Description
•Reports directly to Day Habilitation coordinator.
•Assure all procedures are implemented that program meet federal and state regulations policies and procedures.
•Monitor and Record individual progress to ensure that goals and objectives are met.
•Completes daily individuals progress notes basis according to standard.
•Writes monthly summaries for each individuals served.
•Inspect vehicle prior to use to ensure that they are in safe operating condition.
•Develop individual activities according to Staff Action Plan.
•Responds to all statements of deficiencies issued by the state.
•make sure your area of work is clean and condition to work with participants
•Provide day hab services face to face to assigned individual.
•Responsible to maintains annual trainings update
•Report and complete notables as necessary.
•All other duties as assigned by the program supervisor.

• Skills and Abilities:
• High School diploma/GED
• One year experience working with diverse clients in a human services setting preferred
• Preferably bilingual (English/Spanish)
• Valid NYS Driver's License/Reliable transportation
• A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.
• Physical Demands:
• The position does require occasional standing, squatting, lifting of up to approximately 10 lbs. and frequent sitting.

Salary Description

$16 to $21 per hour
