Empowering People's Independence (EPI) provides support for people with developmental disabilities, epilepsy, and brain injury through progressive, tailored care to improve their health, wellbeing, independence, and social inclusion. Job Purpose: The Community Hab DSP role is primarily responsible for supporting people with intellectual/developmental disabilities as they become further integrated into their communities. You will use your training daily, in addition to your own knowledge and skills to support and guide people toward a healthy and safe lifestyle of their choosing. Essential Functions:· Ensure ongoing health and safety for the person being supported.· Support, train, and guide people as they socialize, make purchases, volunteer, exercise, or engage in other community-based activities of their choosing, guided by the Person-Centered Planning process.· Provide transportation as deemed appropriate for the day’s activities.· Complete online documentation which meets all OPWDD regulations in a timely manner.· Report all incidents to the Program Supervisor and agency QI Team.· Other duties as assigned within scope of practice.Education and Experience:High School Diploma/GEDQualifications:· Valid & current NYS Driver’s License· Must have vehicle as job requires transporting person supported via personal automobile· Familiarity with computers; documentation is web based· No experience necessary, although previous experience in the field of intellectual/developmental disabilities is preferred.Professional Deportment: Consistently demonstrate EPI’s Values:1.
