The Huron County Medical Care Facility in Bad Axe is seeking a full-time Social Worker for our 112 bed Skilled Nursing Facility. The ideal candidate will possess a BSW or MSW with a passion for working in a fast-paced healthcare setting with elders and their families. New graduates are welcome to apply. Role Description
This is a full-time, on-site Social Worker role at Huron County Medical Care Facility located in Bad Axe, MI. The Social Worker will be responsible for providing emotional support, treatment planning, and discharge planning to residents and their families. The Social Worker will also be responsible for coordinating care with other members of the healthcare team and maintaining up-to-date records.
Qualifications
- Must possess a passion for working with elders, teamwork, customer service, and continuous quality improvement.
- Clinical social work skills, including assessment, treatment, and case management
- Knowledge of social work theory and practice, including developmental stages, psychodynamic theory, and family dynamics
- Strong communication and interpersonal skills
- Ability to work effectively with a diverse population, including residents, families, and healthcare professionals
- Excellent organizational and problem-solving skills
- Experience in a long-term care or healthcare setting is preferred
- Bachelor's or Master's degree in Social Work or related field from an accredited college or university
