Human Touch Home Care is currently looking for Registered Nurses for Grand Junction (Mesa County). We value our employees like family. We are a locally operated home care company that has been helping our clients to live their most healthful and fulfilling lives at home in the Grand Junction area.

Essential Job Functions/Responsibilities:

• Completes an initial Oasis E assessment of patient to determine home care needs. Provides a complete physical assessment and history of current and previous illnesses.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
• Educates the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan.
• Develops and implements prior to discharge of the patient.
• Participates in rotational on-call (minimal)
• Provides Case Management of assigned patients by Administrator / DON. Assumes responsibility to coordinate patient care for assigned caseload.
• Delegates appropriate nursing care or visits to LPN, as needed
• Conducts LPN and CNA supervisory visits as assigned
• Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.

Job Requirements:

• Registered nurse with current license to practice professional nursing in the State of Colorado or Compact License
• CPR, AED and first aid certification preferred.
• Minimum of two years nursing experience, at least one of which is in public health or home care nursing is preferred.
• Be organized and detailed, efficient, and accurate in work performance.
• Possess and display excellent customer service skills.
• Possess and display proper communication skills which exude patience and self-discipline.
• Demonstrates excellent observation, problem-solving, verbal and written communication skills; nursing skills per competency checklist.
• Must have reliable transportation and company-required auto liability insurance.
• Willing to travel within 20 miles radius.
