Summary
The Behavioral Health Navigator (BHN) at the Human Services Campus serves as a liaison between homeless Mercy Care members accessing the CES and behavioral health services systems. The BHN will employ a collaborative approach to the implementation of these services to ensure client choice and partner voice, implement program policies in coordination with funder and partners, provide assistance, build and maintain relationships with partner agencies, monitor quality assurance and agreements with participating partner agencies, oversee contract compliance and reporting, and ensure advocacy for best placement for members entering the homeless system.
What You Will Do To End Homelessness
Support a caseload of 14-16 clients referred from Housing Match that are experiencing homelessness and mental health/substance abuse concerns and identify SMI status (Serious Mental Illness) to ensure appropriate connections to clinics, psych evaluations, etc.
Collaboration with Coordinated Entry System (CES), partner agencies and behavioral health system to ensure coordination to support both GMHSU and SMI populations for appropriate connections to care; maintain productive working relationships.
Assist client and coordinate with clinical case managers in obtaining necessary documents for housing, such as: State Identification, Social Security Card, documentation of disability (DVL) and Chronic Homeless Verification Letter (CHVL). If needed assist with obtaining Birth certificates.
Screen eligible SMI members to identify appropriate housing programs based on individual needs and coordinate accordingly.
Utilize the HMIS database for record keeping and maintain a high level of data quality; properly documenting engagements and uploading vital documents needed for housing.
Conduct vulnerability assessments (VI-SPDAT/TAY-SPDAT) as needed.
Identify appropriate housing programs for clients based on individual need and staff with Behavioral Health Coordinator Manager to coordinate the appropriate housing.
Assist HSC staff and Partners to verify enrollment with Mercy Care.
You're An Ideal Fit If
Associate degree in Human Services or a minimum of 2 years’ experience working with mental health service delivery in the state of Arizona.
Experience working with the homeless population a plus.
Knowledge of healthcare and the behavioral health service system and resources is essential.
Must be able to effectively communicate with clients from diverse backgrounds including those who are experiencing mental health, substance use, traumatic, and medical barriers; and must model effective engagement strategies.
Must be able to professionally communicate and maintain product relationships with colleagues and partner agencies; work in a fast-paced environment; work as part of a team; use sound judgment; demonstrate personal responsibility and integrity show initiative; and an ability to work interconnected. Sensitivity to cultural diversity, lifestyle issues is essential.
Effective written and verbal communication skills (Spanish a plus).
Visual acuity sufficient to maintain systems of records and reports containing computers- generated and handwritten
Valid Arizona State Driver’s license as this position will have a significant amount of travel.
Ability to handle sensitive material, maintaining the highest level of confidentiality and display sound
Mobility sufficient to conduct regular duties within a normal office
Ability to lift up to 20 lbs. in order to safely manage office supplies and equipment
