Are you a forward thinking, growth minded leader? Hudson Valley Medical Health Choices is expanding our Palliative Care Program and is seeking critical thinkers and innovators . Palliative care is focused on providing relief from the symptoms and stress of illness with the goal of improving quality of life for both the patient and the family. The successful candidate will be collaborating with Medical Directors and the Vice President of Patient Access for Hudson Valley Hospice to expand our community based palliative care program. This RN Director role will be responsible for establishing, implementing, and evaluating the program’s operational and clinical services in compliance with organizational policies, procedures, applicable regulations, and quality standards; contributing to the organization’s mission and goals to expand patient access to quality palliative care in our region.

Willing to train the right candidate in hospice and palliative care.

· Excellent communication skills, both verbal and written

· Knowledge of information management, including security and confidentiality

· Demonstrate leadership through teamwork and team building skills

· Competency/excellence with computer skills/documentation

· Attention to detail

· Effectively leads and develops staff

· Demonstrates the ability to collaborate productively in all settings

Who we are looking for:

· Current unrestricted NYS Registered Nursing License

· Minimum of 3 years management experience in a health care setting, preferably in hospice operations.

· Care management, case management or triage experience preferred

· CHPN Certification preferred

· Valid NY Driver’s License

Hours: Monday-Friday (8:30am-4:30pm)

Pay: $105,000-115,000 per year
