PRIMARY FUNCTION/PURPOSE:
Provides case management designed to improve individual and/or family quality of life to a specified client group and their families. Coordinates comprehensive daily activities for health care and community-based services, home visits, and assessments. Develops, implements, coordinates, and evaluates health care plans in an outpatient environment; collaborates with other health care providers, community service.
DUTIES AND RESPONSIBILITIES
Coordinates, monitors, and evaluates provider services and care, to include home and/or visits, physician/hospital visits, and therapy visits, for a specified client population.
Documents all client encounters and contracts made on behalf of clients/families; writes progress note and maintain summaries.
Evaluates and documents the progress of individual participation, and makes modifications, as required.
Facilitates client/family access to health care, school, and community resources; assists client/family to develop natural resources and contact social support networks; develops resources as appropriate to meet client/family needs in their community.
Performs a range of associated administrative functions, as appropriate, such as monitoring family budgets, preparing them for in depending living skills.
Travels to outreach locations to meet with clients/families, coordinate services, and attend regular staff and management meetings.
Works with clients and/or families to assess, develop, implement, monitor, and recommend modifications to comprehensive, cost-effective care plans, using a multidisciplinary process.
Performs miscellaneous job-related duties as assigned.
QUALIFICATIONS/PROFESSIONAL EXPERIENCE
Bachelor’s degree in human services. Ability to travel on a regular basis. Knowledge of billing procedures for clinical and social services. Knowledge of clinical and social services documentation procedures and standards. Knowledge of community health services and social services support agencies and networks. Knowledge of nursing and case management theory and practice. Organizing and coordinating skills. Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
POSITION SCOPE:
Provide Case Management services for patients as needed. Track patients from intake and assessment and maintain database of patients who require additional services. Confirm program eligibility of new patients. Schedule and maintain a list of upcoming appointments on our E-Share data base. Confirm patient appointments, make reminder calls and send text messages prior to screening date. Provide one-on-one health education to patients scheduling a screening appointment.
PROBLEM SOLVING: Independently works through routine problems, determining solutions from established standards or procedures.
KNOWLEDGE: Duties require a thorough understanding of principles, concepts and methodology associated with counseling occupation.
FISCAL RESPONSIBILITY: Makes routine low-cost expenditures, as designated by supervisor.
CONTACTS: Works primarily with staff within own program. External contacts are infrequent and limited in scope.
SUPERVISION: Occasionally instructs or assists other employees. Sometimes delegates work assignments to others as directed by supervisor.
OTHER REQUIREMENTS:
Ability to travel locally using public transportation. Required to speak clearly and hear the spoken word well. Required on a regular basis to be prepared to utilize appropriate techniques to ensure consumers safety. Required to utilize near vision to read data and other documents including spreadsheets and reports in printed form or on computer screens. Regularly required to move fingers, wrist and arms in a repetitive manner to operate office equipment.
