The Fedcap Group is Hiring a Case Manager Near Jamaica, NY

Position Summary:

The Case Management Support Program (CMSP) is designed to provide services to Clients referred by the New York City Human Resources Administration (HRA) that have substance use disorders, are in transitional housing, have severe mental illness or other complicating behavioral health issues. Services include CASAC assessment, treatment referrals, and intensive case management (field-based) to meet the complex and immediate needs of the Clients. Applying a client-centered model, Case Managers assist Clients toward achievement of 1) Referral to substance use and/or mental health treatment; 2) Retention in substance use and/or other mental health treatment programs; 3) Income security by maintaining Case Assistance case continuity, gaining and retaining employment, or obtaining Federal Disability benefits.

Essential Job Functions:

  • Advise Clients of their rights and responsibilities regarding Cash Assistance and other benefits.
  • Provide counsel to Clients on the treatment and rehabilitation process.
  • Assist Clients in coordinating a manageable treatment and work schedule and accompany Clients to appointments, as needed.
  • Help Clients to collect documentation for SSI and Housing applications as well as provide advocacy services throughout the application process.
  • Provide community-based services in a variety of settings in a flexible manner, which may include Clients' homes, at treatment facilities, in transitional housing sites, or in shelters; certain services must be offered beyond standard business hours to promote stabilization, income security, and engagement in programs.
  • Provide intake and orientation services to assist Clients with understanding and exploring needs, strengths, interests, aptitudes, experience, capabilities, career goals, and family situations.
  • Create, update, and maintain Clients' Comprehensive Service Plans applying a client-centered model specific to the unique needs of everyone with untreated or unstable substance use, mental health, and/or medical conditions.
  • Address support service needs including, but not limited to: Reasonable accommodations; housing; legal services; medical and/or mental health services; domestic violence; educational, training, and employment services; childcare; and transportation.
  • Facilitate appropriate referrals to WeCARE for comprehensive assessment - also assist Clients to maintain engagement with WeCARE for HRA Clients with medical and/or mental health conditions.
  • HRA or other appropriate employment, education/training, or other services for HRA Clients who are fully employable and for whom treatment is not required.
  • HRA's Home Care Program, housing, or other such services, if determined to be applicable
  • Provide referrals to substance use treatment and/or harm reduction services based on substance use assessment conducted by a CASAC which the Clients are required to attend.
  • Provide referrals for work or other applicable assignments concurrent with treatment if the substance use disorder requires non-intensive treatment.
  • Maintain ongoing face-to-face, telephone, and other collateral contacts to support, encourage, and promote the Client's commencement and ongoing compliance with the requirements of treatment and/or services outlined in the CSP, as well as to assist with mitigating barriers to engagement in services, wellness, and recovery, as well as employability.
  • Identify early warning signs of crisis and/or relapse and conduct timely crisis/relapse interventions.
  • Responsible for capturing and recording employment information and work closely with Retention to monitor newly hired Clients for 6 months.
  • Provide outreach to Clients who have difficulty with engagement to encourage enrollment and successful participation in program services and help address underlying issues to re-engage and make up missed program hours.
  • Stress urgency and ownership, guiding Clients through a learn by doing process during their service planning and delivery.
  • Provide vocational counseling, intervention, and program action planning and delivery including advocating for Clients at treatment programs and work sites.
  • Document activities, participation, compliance, and employment progress in the Electronic Case Management (ECM) systems.

Required Education and Experience

  • Bachelor's degree in social work or a related field, such as psychology, vocational rehabilitation, or occupational rehabilitation from an accredited institution and Graduate Degree a plus.
  • CASAC A .
  • Minimum 2 to 4 years' experience in case management; or training and/or work experience determined to be acceptable to the agency.
  • Bilingual is a plus.
