Housing Matters is Hiring a Supportive Housing Case Manager Near Bemidji, MN

Housing Matters Supportive Housing Program offers supportive housing case management and advocacy for long-term homeless adults, youth and families living in Beltrami County utilizing a Housing First approach. We provide housing navigation, in home case management services, assessing participant needs, and assistance in identifying appropriate and meaningful resources based on client preference. Primary Duties:

The Housing Case Manager is responsible for all aspects of a participants housing including assisting participants with moving into housing, providing ongoing needs assessment, creating personal goal plans, offering support, information, referral, and advocacy on behalf of individuals in our program. This position requires the ability to connect with people who have experienced long-term homelessness and that may have complex mental health, chemical health, or physical health concerns. The Housing Case Manager will also help participants identify issues and obstacles that threaten to undermine their self-sufficiency while working on goal plans around life skills, employment, and education. The Housing Case Manager will work daily doing in-home visits, case documentation of the delivery of services, and work with clients to assist with completing all required paperwork for ongoing eligibility for county, state and federal benefits. The Case Manager will also be responsible for providing transportation to participants, and in establishing and coordinating ongoing care for participants regarding their physical, mental, and chemical health.

Experience:

Position requires a minimum of 1-2 years experience delivering services to homeless populations, or a bachelor’s degree in a related human services field. Excellent verbal and written skills required. Ability to work independently and collaboratively. Preference given to people who have training and experience in de-escalation and crisis intervention. Candidates not meeting the minimum requirements will not be considered.

Housing first implements the following core philosophies:

Utilize Harm Reduction interventions and methodologies to provide client centered services that assist participants in maintaining housing. Utilize skills regarding crisis prevention and intervention. Understand and have the ability to use Motivational Interviewing techniques in daily conversations with participants. Have a working knowledge of Stages of Change and be able to identify and implement this in daily work. Knowledge of supportive housing and how to provide housing and community resources. (Training on Housing First philosophies provided to all new employees)

Other Requirements:

Valid driver’s license with clean driving history.

Written consent to a DHS background check, and ability to pass this is contingent on being offered employment with our agency.

Additional Information:

The following duties are normal for this position.

Be able to successfully build healthy working relationships with all participants through strength-based recognition and a client driven approach to providing services. Assist participants to build upon their strengths and recognize their needs through respect and consideration of their specific situations. Collaborate with the Housing Coordinator to assist participants in the housing application process. Assist participants with moving into new units and moving units when applicable. Assist participants in navigating numerous mental health, chemical health, and physical health services, as well as any other community resources available to our participants. Assist participants in understanding and maintaining public benefits. Meet weekly in home with assigned participants. Keep accurate and up to date information on all participants including but not limited to goal plans, case documentation, and other information as needed. Facilitate relationship building with participant and their landlord or caretaker. Visit and observe participants’ housing to ensure they are safe and that housing is being properly maintained. Work as part of a team to plan and develop resident programs. Attend regular internal and external agency meetings. Attend trainings that increase knowledge of effectively meeting participant needs. Work collaboratively and as a team member and within the organization to implement the mission and plans of Housing First, Beltrami County, MN DHS, and Housing Matters.
