The APRN:
• Working in collaboration with a Medical Director or an IDT physician, acts as a team member for hospice patients in the home care, nursing or assisted living setting.

• Responsible for evaluating and coordinating patient/family needs to enable preventative and supportive care
• He/she may provide attending physician services, symptom management consultation or perform an assessment related to ongoing eligibility for hospice services, in addition to providing evaluation of disease progression and symptom control.
• Utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life for the patient/family unit.
• Collaborates with the multidisciplinary team and the hospice physician to assure that the physical, emotional, and spiritual needs of patient/family are met.

Education/Professional:

1. Master’s Degree in Nursing or a related field.
   2. Current and active APRN license in the State of Florida with prescriptive authority.
   3. Minimum 3 years of experience as an RN providing recent direct patient care in an acute care setting.
   4. Previous assisted living or long term care experience strongly preferred.
   5. An equivalent combination of professional experiences and education may be considered as meeting the
   educational/professional requirements.
