Primary tasks, duties, and responsibilities:
1. Promotes and practices the Hospice mission and core values, following all policies and procedures of Tidewell Hospice.
2. Assists the Clinical Director with the scheduling of home team staff, adapting to the patients’ needs while ensuring patient care efficiency, effectiveness, and satisfaction.
3. Responds to patient and family phone calls, evaluates the need, provides recommendations based on the established plan of care and assists with assigning colleague visits when necessary.
4. Directly supervises all Hospice Aides on the team including supervision, coaching, mentoring, corrective action as needed including completion of annual performance appraisals.
5. Recognizes, reinforces, and enhances the role of all disciplines in the IDG by acting as a role model, mentor and first line nursing resource. Provides education to IDG members regarding nursing aspects of patient/family care.
6. Collaborates and communicates effectively with associate medical director, clinical teams, admission team, liaisons and all colleagues residing at the site.
7. Assists the Clinical Director with facilitating the Interdisciplinary Group review process in collaboration with the team physician.
8. Ensures compliance with all CHAP, State, Federal, and local government regulations; maintains a strong position against fraud and abuse.
9. Assists the Clinical Director with ongoing review of patient documentation to ensure the documentation is complete, accurate and timely and to verify that the electronic medical record system (EMR) is being utilized appropriately.
10. Supports the Clinical Director with monitoring patient ratios for team members, maintaining necessary staffing with census fluctuations.
11. In collaboration with the Clinical Director, participates in Quality Assessment Performance Improvement strategies and initiatives to assure quality of care and improved outcomes.
12. Assumes responsibility for the home team in the absence of the Clinical Director, providing oversight of the team functions related to staffing, patient/family needs and any other required duties that emerge.
Educational/Professional:
1. Must possess current and active Florida licensure as RN.
2. Minimum of 2 years recent community-based hospice or home health experience required.
3. An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
